refactor var: ADMIN -> INITIAL_ADMIN, dyn DOMAIN
- specify clearly the ADMIN vars are when initialization, also add the password
- also allow override of domain for test instances

- generic: create test users
(default vars are 2fa=true, emails=true, test_users=false)
